Characteristics of pediatric SARS-CoV-2 infection and potential evidence for persistent fecal viral shedding

  • Mild and atypical presentations of the infection in children may make it difficult to detect. Indeed, all the patients reported here were found through screening of suspected cases.
  • We also observed positive real-time RT–PCR results in rectal swabs in eight out of ten pediatric patients, which remained detectable well after nasopharyngeal swabs turned negative, suggesting that the gastrointestinal tract may shed virus and fecal–oral transmission may be possible. Indeed, fecal–oral transmission does exist with other respiratory viruses. These findings also suggest that rectal swab-testing may be more useful than nasopharyngeal swab-testing in judging the effectiveness of treatment and determining the timing of termination of quarantine.

Children with Coronavirus Disease 2019 (COVID-19) A Review of Demographic, Clinical, Laboratory and Imaging

  • Most research findings regarding adult cases, which are not always transferrable to children. In this review, we evaluated the demographic, clinical, laboratory and imaging features from 2,597 pediatric patients of COVID-19 that reported recently.
  • We found that even lymphopenia was the most common lab finding in adults, it infrequently occurred in children (9.8%).
  • Moreover, elevated creatine kinase MB isoenzyme (CK-MB) was much more commonly observed in children (27.0%) than that in adults, suggesting that heart injury would be more likely to happen in pediatric patients.

Clinical and CT features in pediatric patients with COVID‐19 infection: Different points from adults

  • In a clinical setting, pharyngeal swab COVID‐19 virus nucleic acid detection is an important basis for diagnosis. However, due to the limitation of sampling materials, especially in the early stage of the disease, the positive rate is relatively low. Therefore, early diagnosis is of great significance for further rational management and clinical treatment of children.
  • Moreover, since most of the pediatric patients are mild cases, plain chest X‐ray often fails to show the lesions or the detailed features, leading to misdiagnosis or missed diagnosis. Therefore, early chest CT examination is very necessary. In some pediatric patients with a negative nucleic acid of COVID‐19 virus, we managed them as suspected patients according to the typical lesions shown on chest CT, which could provide evidence not only for early treatment to the children but also for effectively isolating the source of infection.

  • In conclusion, our study shows that paediatric patients with COVID-19 have a simple transmission mode, either by close contact with infected adults or by exposure to epidemic areas. Although fever, dry cough, and mild pneumonia are common manifestations, nearly half of patients have neither obvious symptoms nor abnormal
    radiological findings. The proportion of asymptomatic cases indicates the difficulty in identifying paediatric patients without clear epidemiological information.
  • Although all paediatric patients in our cohort had mild or moderate type of COVID-19, the large proportion of asymptomatic children indicates the difficulty in identifying paediatric patients who do not have clear epidemiological information, leading to a dangerous situation in community-acquired infections.

  • Children with COVID-19 infection may be asymptomatic or have fever, dry cough, and fatigue, with a few upper respiratory symptoms, including nasal congestion and runny nose; some patients have gastrointestinal symptoms, including abdominal discomfort, nausea, vomiting, abdominal pain, and diarrhea. Most infected children have
    mild clinical manifestations, and the prognosis is good. Most of the pediatric patients have recovered within 1-2 weeks after onset.
  • SARS-CoV-2 is highly contagious. Although the number of reported pediatric patients is small at this point, children are vulnerable to the infection. The importance to increase awareness, reinforce infection control measures, and perform health management within the families can never be overemphasized.

  • We report an asymptomatic child who was positive for a coronavirus by reverse transcription PCR in a stool specimen 17 days after the last virus exposure. The child was virus positive in stool specimens for at least an additional 9 days. Respiratory tract specimens were negative by reverse transcription PCR.
  • Our finding of multiple positive stool specimens in this case similarly raises the concern that stool from COVID-19 patients might serve as another vehicle for virus transmission.

  • Children at all ages appeared susceptible to COVID-19, and there was no significant gender difference. Although clinical manifestations of children’s COVID-19 cases were generally less severe than those of adults’ patients, young children, particularly infants, were vulnerable to infection.
  • Furthermore, this study provides strong evidence for human-to-human transmission.
  • To the best of our knowledge, this is the first retrospective study on the epidemiological characteristics and transmission dynamics of children’s COVID-19 in China. As most of these children were likely to expose themselves to family members and/or other children with COVID-19, it clearly indicates person-to-person transmission.

  • Analyses of how cases are detected, and use of data on individuals exposed but not infected, indicate that infection rates in young children are not lower than the population average (even if rates of clinical disease are).
  • Moreover, children are at a similar risk of infection to the general population, although less likely to have severe symptoms; hence they should be considered in analyses of transmission and control.
  • These results shed further light on how SARS-CoV-2 is transmitting, how severe it is, and how effective control measures can be in specific contexts. We provide a key piece of evidence supporting intensive contact tracing and highlighting that children might be an important target for interventions aimed at reducing transmission, even if they do not get sick.
  • Notably, the rate of infection in children younger than 10 years (7·4%) was similar to the population average (6·6%).
  • Moreover, children are at a similar risk of infection to the general population, although less likely to have severe symptoms; hence they should be considered in analyses of transmission and control.

  • We found no differences in seroprevalence between children/young-adults (5-19 years old) and middle age adults, with those older than 50 years having a significantly lower seroprevalence than 20-49 year olds. Although children present typical COVID-19 symptoms far less frequently than adults, these results provide support to emerging research showing that they indeed get infected at similar rates. This should be considered in view of increased concerns about severe inflammatory syndromes in children that could be COVID-19-related, and of the worldwide debate around opportunity and modality of school re-openings.

  • Our results do not necessarily indicate that reopening school is safe regarding the potential of re-emergence of Covid-19 spread since various rates of transmission are expected to occur also in school and therefore reopening school should be practiced with caution and with close monitoring of Covid-19 spread.

  • Zusammenfassend sprechen die folgenden Faktoren dafür, dass Kinder – wie bei anderen respiratorisch übertragbaren Erkrankungen – relevant zu einer Verbreitung von COVID-19 beitragen: (1) Kinder haben häufig einen asymptomatischen oder sehr milden Verlauf, und werden dementsprechend oft nicht als SARS-CoV-2-Infizierte erkannt; (2) Asymptomatische und präsymptomatische Übertragungen spielen im aktuellen Infektionsgeschehen prozentual eine wichtige Rolle; (3) Asymptomatische und präsymptomatische Übertragungen können ohne Schutzmaßnahmen im Alltag nur schwer verhindert werden; (4) Vor allem jüngere Kinder können sich nicht in vollem Umfang an kontaktreduzierende und Hygienemaßnahmen halten. Es besteht damit die Gefahr, dass sich SARS-CoV-2 effektiv unter Kindern und Jugendlichen in Betreuungs- und Bildungseinrichtungen ausbreitet. Auf Grund der verschiedenen und engen außerschulischen Kontakte ist zudem von einem Multiplikatoreffekt mit Ausbreitung in den Familien und nachfolgend in der Bevölkerung auszugehen.
